Record the date and time of each item, the physician or nurse's name, what is being done, and why they are doing what they plan to do. If something goes wrong down the road, you are not going to have a clear mind to go back and try to recall all these things, and such a diary will be a key component if your malpractice case ends up in court.
The burden of proof is on you, the patient or the family of the patient, which is where this notebook or diary is going to be needed.
