Hair transplant surgery is one of the branches of cosmetic surgery that has been revolutionized in the last twenty years. Medical treatment for hair loss has a long history and as far as we know it goes back to the era of Ancient Egypt. Hair transplant using surgical techniques has been around for more than hundred years with different levels of success. In 1939 a Japanese dermatologist invented a technique for transplant of skin containing hair to the burned area on the scalp to create some hair coverage for the area. That procedure could probably be called the first ancestor of today's refined hair transplant. The concept of moving skin and hair was adopted by Dr. Orentreich in 1952 in United States for treatment of male pattern baldness.

The old technique of hair transplant allowed surgeons to remove hairy skin from the back or permanent area of the scalp and implant it to the frontal balding area of the scalp. This surgery used to produce plugs of hair in the frontal area to add to the number of hair and make baldness less noticeable. These plugs could create islands of skin with high density hair, when patients lost more hair in the areas surrounding the plugs. Many of the patients, treated with these techniques, had to do revision surgery to remove the unnatural plugs.

Unsatisfactory results of the old procedures made hair transplant surgeons cut grafts to smaller sizes, developing mini and micro-grafts. That was a start and a good to produce more normal look. Despite of long period of ignoring natural grouping of hair follicles, the process of refining follicular units and discovery of Follicular Units was inevitable in about twenty years ago. Follicular Unite Transplant (FUT) set a new standard in hair restoration surgery. FUT has made the dream of natural looking hair transplant, a reality and is currently the only standard of care in hair transplant surgery.

Hair transplant surgery has done its evolution from very larger hair grafts (plugs), to mini and micro-grafts and eventually follicular unite transplant in less than three years. FUT, which allows harvesting of hair grafts in their natural occurring groups, can produce results that are not even detectable by hair specialists with naked eyes.

Other advance in the techniques and instruments of hair transplant made larger surgeries possible and facilitated transplant of maximum number of grafts in only one session. The emphasis on natural looking surgery and elimination of scars advanced the methods to minimize the scar of the donor area. Pharmaceutical discoveries in production of the new medications for prevention of hair loss were also used in conjunction with hair transplant with synergistic effects in the last few years.

There are still other techniques on horizon for hair restoration. Hair multiplication and hair cloning have been hot topics in last two years. The idea of producing unlimited number of hair from a single hair follicle is being researched and could be an option for patients with limited hair supply in the future. Expansion in knowledge of human genome and the advanced technology for gene expression are among other means that may help us to keep our heads full of hair.
